Frequently Asked Questions
How many employees do I need to have a group plan?
Generally, any employer that has at least 2 full time employees can qualify for a group health, dental or vision plan.
Do I need to wait until the government’s “open enrollment period” to buy a group health plan?
No, you can start a group health plan for you and your employees any time of the year. The government’s “open enrollment” period typically only applies to the individual insurance market. Your group plan will have an “open enrollment period” every year (generally during the month you first started the plan), but it does not need to mirror the governments set “open enrollment” period for individual plans which is around January.
Can I have more than one plan?
Yes, you can offer multiple plans for your employees. This gives your employees different options and will allow them to pick the best plan for their individual needs.
How much do I have to pay as the employer?
In California, an employer must pay at least 50% of the employee-only premium to have a group health plan. Employers do not need to contribute toward their employee’s dependent premium. Also, there are many dental and vision plans that can be offered on a voluntary bases that do not require any employer contribution.

There are new requirements on waiting periods, required coverages, and how rates will be determined. It is more important than ever to have a professional agent working to help you navigate these in health care. Please call today with specifics to see how the ACA will affect your unique business.

Networks are Important
Another important consideration when buying a group health insurance plan is to buy a plan that is well accepted where your employees live. Most benefits are based off of your company’s business ZIP Code. However, you may have employees that live out of the area or even out of state. Will the health insurance plan you purchase work for your out-of-area employees? What if you have an HMO plan or an EPO group health plan, but your employees live in a PPO area where there is no HMO or EPO network? These questions are very important to address before you choose to purchase a plan. Ross Rogers Insurance Services can help you understand the options, and make sure the plan you purchased will work well for your business and all of your employees!
PPO
A PPO plan (Preferred Provider Organization) is a health plan that is considered “freedom of choice”, meaning your employees can go to any doctor that accepts that health insurance plan. In most cases, your employees will not need a referral form their primary care physician. The insurance company has already negotiated the prices for procedures with in-network doctors. When they go to an in-network doctor, all services will be paid/applied to the in-network benefit of their plan. Because of this, the doctor must take this amount as payment in full, and is not allowed to “balance bill.”
Many PPO plans also have an “out-of-network” benefit, meaning that if an employee goes to a doctor that isn’t inside the PPO network, they can still have some of the expense applied towards their out-of-network benefit. It is important to note that going out-of-network could result in being “balance billed” by the provider. When you go out of your insurance company’s network, most insurance companies will only pay a portion of what they would have paid an “in-network” doctor. This could result in the “out of network” provider “balance billing” the employee for the rest of the costs not covered by the insurance. Again, in-network doctors are not allowed to balance bill.
HMO
Another type of health insurance plan is called an HMO (Health Maintenance Organization) and is more common in larger, metropolitan areas. HMO plans offer a wide range of healthcare services through a network of providers who agree to supply services to members. With an HMO plan, employees will need to pick a primary care physician (PCP). The PCP will typically take care of most of their health care needs. If they want to see a specialist, they will need to obtain a referral from their PCP. Unlike PPO plans, HMO’s have little to no out-of-network benefit. Therefore, if they choose to go to a doctor or facility that in not part of the HMO, it may not be covered by the insurance.
EPO
Another type of health insurance option is called an EPO (Exclusive Provider Origination). Many of these plans, like an HMO, do not have any out-of-network benefit. It is very important when purchasing an EPO plan to verify that the providers (Hospitals, Doctors, Specialists, etc.) that are important to your employees are contracted as “in-network” by your insurance companies EPO network. There are a few ways to do this. One, you can do a provider search of the health insurance company’s EPO network on their website, or you can contact Ross Rogers Insurance Services and we can look this up for you.
